I．はじめに

　脳室ドレナージ，脳室シャント術など，脳室穿刺を要する術式は脳神経外科臨床における日常処置として広く行われ，また穿刺に起因する脳病変についても早くから指摘のあるところである．しかしながら，合併症として穿刺直後の出血を除く亜急性出血の報告は少なく，特に遅発性急性出血の報告は稀であり，わずかに重症頭部外傷例に脳室トランスデューサーを設置し出血を来たした例^4,8,12）をみるのみである．今同われわれは聴神経腫瘍摘除術を施行するにあたり，頭蓋内圧コントロールの目的にて術中脳室ドレナージを行った症例に，術後14日目，脳室穿刺部位を出血源とする急性脳内出血を来たし，出血は硬膜下のみならずburr-holeを通して皮下にまで及んだ興味ある症例を経験したので，若干の文献的考察を加え報告する．

A case of delayed apoplexy following a traumaticpuncture of the left lateral ventricle is reported. Thepatient was a 55-year-old man, who had no history ofhypertension but suffered from hearing difficulty of theleft. Neurological and neuro-racliological examinationshowed a cerebellopontine angle tumor on the left. Sub-occipital craniectomy and total removal of an acousticschwannoma (Antoni A type) were performed under controlof the intracranial pressure with ventricular drainage, whichwas placed through an occipital burr-hole at the beginningof operation.
