はじめに

　1961年より1975年9月までに東北大学脳神経外科では，嚢状脳動脈瘤直接手術1,000例を経験し，入院中死亡率は6.1％であった．この直接手術1,000例のうち多発性脳動脈瘤162例を除き，術後脳血管写を施行し，判定に耐えうる脳血管写の得られた症例は578例であった．この578例中動脈瘤陰影が多少とも残存していた41例について検討し若干の文献的考察を行ったので報告する．

Among the 1,000 cases of direct surgery on saccular aneurysm treated in our clinic from 1961until September, 1975, pre- and post-operative angiograms of simpie aneurysms were obtained in578cases and an investigation was made on those cases showing a part of the aneurysm remaining postoperativery. Forty-one cases were found to have residual aneurysms, and 19 of these cases were further treated by complete muscle wrapping or wrapping subsequent to neck treatment. That is, despite sufficient surgical treatment, residual aneurysms were seen in 19 cases.
