Ⅰ．はじめに

　近年，脳動脈瘤の治療は，直接手術の普及が著しく，内頸動脈結紮手術のような間接的手術を行うことは少なくなっている．ことに椎骨脳底動脈系の動脈瘤に対し，近位動脈結紮を行う手術は従来も決して多くはなされていなかった．しかし最近Drake^6）が巨大椎骨脳底動脈瘤に対し，近位動脈結紮を行った21例の報告は注目に価する．後頭蓋窩の動脈瘤は巨大になるものも多く^6，8，9），またことに椎骨動脈合流部附近の動脈瘤は側頭経路でも後頭下開頭でも接近不可能な部位である^19）．直接手術法としては，経斜台法^16）が考えられるが，巨大動脈瘤に対して十分な視野をうることはむずかしいと思われる．Hammon^9）は乳様突起まで達する後頭下斜接近法を提唱しているが巨大動脈瘤ことに後方へ向うものでは困難と思われる．

　著者らは，巨大な椎骨動脈合流部動脈瘤の1例を経験し，椎骨動脈結紮術を行い，好い結果をえたので報告する．

A 58-year-old woman suffered diplopia, painful hypesthesia in the right half of the face and mild ataxia. Angiography demonstrated an aneurysm, 2 cm in diameter, arising from the vertebrobasilar junction pointing right laterally and posteriorly. The aneurysm was visualized by both right and left vertebral angiograms, but opacification of the aneurysm via the left vertebral artery was denser, and the axis of the aneurysm extended along the course of the left vertebral artery. Therefore, the blood stream from the left vertebral artery was considered to be more responsible for the aneurysm.
