頭蓋底，後頭蓋窩および全脊髄蜘蛛膜下腔への広範な播種性転移を示した第Ⅲ脳室後部Malignant teratomaの1剖検例

An autopsy case of malignant teratoma in the posterior third ventricle with disseminated metastasis to the basal cistern, posterior fossa, and the whole spinal subarachnoid space

　先天性腫瘍の一つに分類されている頭蓋内奇形腫は，小児期および思春期の頭蓋内腫瘍としては決して稀なものでなく，とくに日本においては，松果体部腫瘍の中でも，いわゆるtwo cell pattern pinealoma（germinoma）についで多く見られる腫瘍である。しかし奇形腫は，いわば過誤腫とみられているごとく一般には良性腫瘍で発育は緩徐であり，転移することは稀であるとされている。一方松果体奇形腫は臨床的に性早熟を伴う症例が多いことから性ホルモンとの関係が古くから議論され，松果体部腫瘍と内分泌機能に関する報告は多い^{1,4,8〜11,15）}。こうした意味からも，この部の腫瘍は種々の興味ある問題を含んでいるが，われわれは臨床的には性早熟を示さず，早期再発と多彩な神経症状を呈し，剖検によりCSFを介して頭蓋底，後頭蓋窩，および全脊髄蜘蛛膜下腔へ転移したmalignant teratomaの1例を経験したので若干の文献的考察を加えて報告する。

A case of malignant teratoma in the posterior third ventricle with wide disseminated metastasis through the CSF pathway to the subarachnoid space of posterior fossa and to the whole spinal cord was reported.

A male student aged 14 years, who had signs of increased intracranial pressure and decline of the conciousness level, was admitted to our clinic. Neuroradiological studies revealed highly vascu-larized tumor in the posterior third ventricle.
