I．はじめに

　今までに報告された"いわゆる"後交通動脈動脈瘤の多くは，内頸動脈—後交通動脈分岐部に発生し，"真の"後交通動脈動脈瘤は非常に稀である．今回，われわれは，後交通動脈本幹より直接発生し，外転神経麻痺を呈した嚢状動脈瘤を経験したので報告する．

Most of "so-called" posterior communicating artery aneurysms previously reported, originated from the internal carotid-posterior commuicating junction. Aneurysms arising from the posterior communi-cating artery itself are very rare. The abducens nerve palsy caused by cerebral aneurysm is also very rare. We are reporting a case with the saccular aneurysm arising directly from the distal half of the posterior communicating artery presenting the abducens nerve palsy.
