パーキンソン病の主として初期治療指針について述べる。若年者で痴呆のない患者ではDA作動薬での治療導入がもっとも推薦されるが，重症者や高齢者，痴呆合併例ではL-DOPAによる治療導入が推薦される。DA作動薬単独治療で症状コントロールがよい患者ではその療法を継続し，L-DOPAで治療導入した患者でも早期にDA作動薬併用に移行すべきである。長期経過例の運動合併症についてはL-DOPAの効果に依存した治療をあらため低用量のL-DOPAとL-DOPA減量に見合う十分量のDA作動薬の併用が基本となる。治療方針の決定には患者個々の現時点における問題症状をよく理解することと，治療薬剤の特性を理解することが大切である。

This article mainly describes therapies administered during the early stages of Parkinson's disease. In the case of younger patients without dementia, initiation of therapy with dopamine receptor agonists (DA agonists) is recommended. On the other hand, in the case of patients in advanced stages, elderly patients or patients with dementia, therapy with levodopa is recommended. Patients whose symptoms are well controlled by DA agonist monotherapy are not administered with the levodopa. In the case of patients in whom levodopa was introduced at the start of the therapy, early adjunction of a DA agonist is recommended.
