脳虚血と一酸化窒素

Cerebral ischemic damage and nitric oxide (NO)

はじめに

　内皮依存性に血管が拡張する現象が発見され^1），その後まもなく内皮由来血管拡張因子（EDRF，endothelium-derived relaxing factor）が一酸化窒素（nitric oxide，NO）と同定された当時^2），脳血管調節の研究に携わっていた筆者は，大変興奮したことを覚えている。その後間もなく筆者らは，NOが脳血管において拡張性に重要な役割を果たしていることを報告したが^3），その時には血管反応性の立場でしかNOを促えていなかった。しかしその直後よりNO合成酵素（NO synthase，NOS）が血管内皮のみならず，脳実質内神経細胞，グリア細胞，血管周囲神経など生体内のいたる所に存在することが次々と報告されるにおよんで，NOの役割の複雑さに戸惑いを感じたのは，筆者のみではないと思われる。本稿では，最近の大きなトピックの一つである脳虚血におけるNOの関わりにつき，主に細胞損傷の立場より今までの報告を振り返り，その膨大な情報を整理して行きたいと思う。NOと脳循環調節については筆者の他の総説^4,5）を参照して頂ければ幸いである。

　Nitric oxide synthase (NOS) is basically divided to constitutive NOS (cNOS) and inducible NOS (iNOS) based on their activation manner including their Ca2+ dependency. They are widely distributed in practically every constituent of the brain tissue such as neurons, glial cells, endothelial cells, vascular smooth muscle, perivascular nerve fibers, macrophages, leukocytes and so on.

　From the view point for strategies against ischemic damage, we may divide basic biological roles of NO into two categories, foe and friend.
