まえがき

　高齢者社会に入りつつあるわが国では，四肢のしびれ感，手指の巧緻障害や歩行障害を訴えて来院する患者は少なくない。そのもっとも多い原因は，いわゆる頸椎症性脊髄症（以下，頸部脊髄症）といわれる脊椎の変性疾患である。その治療には保存的治療が優先することは言うまでもないが^40），脊髄症を呈した症例ではその効果も一時的なことが多く，結局，観血的治療に至る場合が多い^28）。

　ここに，頸部脊髄症に対する手術の適応と成績ならびにその問題点につき述べる。なお頸部脊髄症として，頸部の変形性脊椎症（CS）のほかに，椎間板ヘルニア（CDH）および後縦靱帯骨化症（OPLL）による症例も含めて論述するが，本論文には頸椎症性神経根症や頸椎症性筋萎縮症^43）は含めない。

Methods, indication, results and problems of surgical treatment of cervical myelopathy due to cervical spondylosis (CS), cervical disc hernia (CDH) and ossification of the posterior longitudinal liga-ment (OPLL) are discussed in this paper. Surgical treatment is indicated in patients with myelopathy, the symptoms of which can not be relieved by conservative treatment. However, surgery should be perfor-med earlier in patients whose cervical spinal canal is, in the cervical lateral film, less than 14 mm in width, and whose lower extremity symptoms are severe at the time of their first visit to the hospital.
