男子神経因性膀胱の排尿障害治療における解剖学的な経尿道的前立腺切除術の経験

EXPERIENCE ON THE ANATOMICAL AND AGGRESSIVE TRANSURETHRAL RESECTION OF THE PROSTATE IN THE NEUROGENIC BLADDER DYSFUNCTION OF THE MALE PARAPLEGICS

緒言

　神経因性膀胱による排尿障害（ここでいう排尿障害とはfailure of emptying bladderの意味である）の原因の1つとして尿道括約筋の絶対的，あるいは相対的な抵抗の増加が注目され，近年その治療の主眼も如何にしてその減弱を計るかに傾いてきている^1）。Α-遮断剤であるPhenoxybenzamine（POB）に代表される薬物治療法もその一つであるが，その実際の効果は理論的根拠^2）から期待されただけのものが全例に上がつているとはいえないのがわれわれの経験である^3）。かかる場合の治療法として間歇自己導尿法^4）ももちろんであるが，これとて決して患者をカテーテルフリーの状態としているわけではない。一方，McNeal^5），Dröesら^6）の報告に代表されるように膀胱頸部，尿道括約筋の構築に関する最近の知見は旧来のものと異なつてきており，またRazら^7）によれば，これら括約筋の一部を構成する前立腺，あるいはその外科的被膜内にもα-adrenergic receptorが存在するとされている。これら最近の尿道括約筋の解剖学的，生理学的知見に基づいて従来の方法とはまつたく異なるanatomicalかつaggressiveな経尿道的前立腺切除術（以下TUR-Pと略す）を薬物治療を含めた保存的治療に抗する脊髄損傷患者（以下脊損と略す）に施行し著効を得たので以下に報告する。

A modified method of transurethral resection for the treatment of neurogenic vesical dysfunction of the male paraplegics was described. Not only all of the inner prostate gland but also the part of surgi-cal capasule of the apical region andouter prostate gland were thoroughly resected with Storz resectoscope. This anatomical and agrressive transurethral resection of the prostate was employed in 17 paraplegics who were either refractory to conservative managements including the use of α-adrenolytic drugs or reafflicted with poorly balanced bladder associated with progressive deteriolation of the upper urinary tract.
