63歳，男性。主訴は血精液症。超音波検査，膀胱鏡，精管造影，経会陰式囊胞造影で前立腺囊胞と診断した。その後外来通院中に，足を組むと初期血尿をたびたび繰り返した。経尿道的囊胞開放術を試みることとした。これに先立ち精丘奥の前立腺部尿道の小孔から極細径膵管ファイバースコープを用いて腺腔内観察を行った。管腔内に数個の結石がみられ，さらに進めると囊胞内と思われる広い空洞に至った。結石による貯留性囊胞と考えられた。

We report a case of retention cyst of the prostate in a 63-year-old man. He visited our department because of hematospermia and gross hematuria. Transrectal ultrasonography and magnetic resonance imaging showed a cyst,2.5cm in diameter,in the posterior part of the prostate. After observing the inside space of the cyst by using a fiberscope,we performed transurethral resection of the cyst wall because of persistent urinary symptoms. The superfine fiberscope with an outer diameter of 0.8mm was introduced through the cavity beside the urethral caruncle. On the way to the cystic space several stones were encountered. The fiberscope could be advanced slowly into the inside space of the cyst. We diagnosed retention cyst caused by the stones of the prostate. Postoperatively he had no complaint of urinary symptoms.
