Café au Lait Spotsを伴えるNevoxanthoendotheliomaの1例並びにNevoxanthoendothelioma本邦報告例50例とXanthoma Disseminatum同23例との統計的観察—両症の鑑別に関して

ON A CASE OF NEVOXANTHOENDOTHELIOMA ASSOCIATED WITH CAFE AU LAIT SPOTS AND REVIEW OF 50 CASES OF NEVOXANTHOENDOTHELIOMA AND 23 CASES OF XANTHOMA DISSEMINATUM REPORTED IN JAPAN

　1912年McDonagh^1）がjuvenile xanthomaと異なる黄色腫5例を集めて報告し，nevoxanthoendotheliomaなる名称を提起して以来，その発症病理に関して多くの紛糾を呼びながらも，今日その独立性はほぼ容認されるところとなつている．しかしながら，本症のclinical entityとして挙げられている諸点は，しばしばxanthoma disseminatumと相通ずる事が多く，夫々定形像をとる時には両症の鑑別にさして困難を感ずることはないにせよ，境界線上の臨床像をとる時にはその症例の帰属に困却することがある．著者は先にトルコ鞍の拡大を伴つた本症の1例を本誌に報告したが，今回自験第1例^2）と臨床的に聊か趣きを異にする本症を経験した．第2例の診断にあたつて，両症の画然たる鑑別の手掛りを得るために，自験2例を加えたnevoxanthoendothelioma本邦報告例^2〜41）51例およびxanthoma disseminatum同23例^42〜61）について統計学的に比較検討し，ある程度の結果を得たので，ここに報告する．

A case of 2-year-old boy with cali au lait spots on his trunk who manifested multiple soy-bean-sized yellow papules on his scalp and back since 6 month old was reported.

Histologic examination of papules showed xanthomatous stage of Nevoxanthoendothelioma.

Differential points of Nevoxanthoendothelioma (NXE) and Xanthoma disseminatum (XD) by statistic analysis were as follows:

1) Onset after 10 year old is XD.

2) A case associated with cale au lait spot is NXE.

3) A case clinically showing teleangiectasia is NEX.

4) Solitary type is NXE.

5) A case histologically showing endothelial proliferation of blood vessels is XD.

6) A case associated with diabetes incipidus is XD.

7) A case having a tendency to cure within 3 years is NXE.

8) A case involving extremities may be XD and a case involving scalp, face and trunk may be NXE.
