皮膚症状を伴つた骨好酸球肉芽腫

EOSINOPHILIC GRANULOMA OF BONE WITH SKIN MANIFESTATION

　骨好酸球肉芽腫（Eosinophilic granuloma of bone，以下E-Gと略す）は良性の経過をとる限局性の単発性または，多発性の破壊性骨病変を特長とする疾患で，1940年，Ohtani ＆ Ehrlich^1），Lichtenstein ＆ Jaffe^2）によつてそれぞれ別々に独立疾患としての研究の基礎が築れたものである．その後，E-G，Hand-Schullcr-Christian病（以下H-S-Chr病と略す），Letterer-Siwe病（以下L-S病と略す）3疾患の関係が問題となり，これらを同一原因により発生する疾患群としてとらえ，Histiocytosis X^3）またはReticulogranuloma^4）などの総括名の提唱がなされているが，なお異論^5〜10）もある現状である．おれおれは，最近，2歳3カ月の男児の側頭骨に単発し，病理組織学的にも典型的なE-Gで，原発巣を手術的に治癒せしめた後に，皮疹を併発した1例を経験し，電顕的検索をも行つたので，これを報告するとともに，主としてE-Gにみられる皮疹の意義と，これと関連してE-GのHistiocytosis Xの中における位置づけについて2,3の文献的考察を行いたいと思う．

A case of this disease in the left temporal bone in a 2-year-and-3-month-old boy with the disseminated papules after the extirpation of the lesion was reported.

Histologic pictures of the eruption resembled those of Letterer-Siwe's disease and were different from the findings of the bone.

Electron microscopic findings of the histiocytic cells of the eruption showed almost the same character-istics as those in Hand-Schuller-Christian's and Letterer-Siwe's disease. These results from histologic and EM studies strongly suggested that these three diseases belonged to the same disease entity.

When the eosinophilic granuloma is accompanied with the disseminated papules, it should be regarded as a sign of the progression and exacerbation. In such an instance the chemotherapy including vinblastin sulfate and adrenocortical steroid therapy should be tried.
