軟部好酸球肉芽腫の1例

AN EOSINOPHILIC GRANULOMA, A CASE REPORT

　I．はじめに

　好酸球肉芽腫と総称されているものは，好酸球浸潤を伴つた肉芽腫が骨，皮膚，胃腸管，皮下軟部組織に発生するもので，これらの病因についてまだ不明の点も多く，治療法にも論議の多いところである。また本症が比較的まれな疾患であるとされ，本邦において，数十例の報告があるに過ぎず，それも外科，皮膚科領域からの報告が主で耳鼻科領域からの論文はきわめて少ない。今回私どもは約2年間にわたり左耳介周囲に発生した好酸球肉芽腫を経験し，腫瘤組織には線維増生を伴い，かつ広範な好酸球浸潤をみとめ，放射線治療，副腎皮質ホルモン治療も無効で手術的処置により治癒した1例を報告し併せて文献的考察を述べる。

　A woman, aged 53, complained of a swelling around the left earlobe which gradually enlarged in size within the last 2 years. The swelling extended posteriorly, anteriorly and inferiorly around the ear lobe. Pathological examination revealed it to be an eosinophilic granuloma.

　The tumor was treated with Co⁶⁰ irradiation with the total exposure of 5,200 r. The respose was immediate with a complete remission; the patient was discharged from further medical care.

　But, soon thereafter recurrence of the growth occurred. The growth became gradually enlarged, even larger than originally, in spite of administration of steroid therapy in combination Co⁶⁰ irradiation.

　The growth was finally exterpated by surgical intervention. Since then there has been no recurrence at present 1 year after the surgery.
