Dermolipoma

DERMOLIPOMA

　Dermoidは皮膚組織が胎生期に断裂，迷入して生じた一種の分離腫で，その多くは嚢胞を形成するdermoid cyst （皮様嚢腫）であるが，ときに充実性で嚢胞のみられないことがあり，このようなものをsolid dermoidという^1,2,3）．このうち表皮や皮膚付属器管以外に脂肪組織に富むものはdermolipoma^1,3,4）あるいはlipodermoid^5,6）とよばれ，角膜輪部や結膜下などの眼科領域に発生しやすい．皮膚科医がdermoid cystをみる機会は少なく^7,8），ましてsolid dermoidやdermolipomaに関しては皮膚科書にも記載なく，皮膚科領域での報告もみられない．

　われわれは眼瞼に発生したdermolipomaの1例を経験し，この結節が皮膚類似組織からなるため皮膚組織病理学的にも興味あると考えたので，文献的考察も加えてここに報告する．

A 41-year-old man noted a subcutaneous nodules on the left upper eyelid 5 years ago. It was an egg-shaped, 1cm in diameter, elastic hard tumors without fixation to the surrounded structures.

Histologic specimen was composed mainly of fat cells, mixed with the keratotic cyst-like structures, hair follicle-like tissue, nests of basaloid cells, and foreign body granuloma.

Dermolipoma is a kind of dermoid cyst, few cases of which have been reported in the ophthal-mology.
