当科における甲状腺癌の治療成績—リンパ節転移（N）が予後因子か？

　はじめに

　甲状腺癌の予後を左右する危険因子を初診時の患者の状態でスコア化する試みがなされており，EORTC score^1）およびAGES score^2）がその代表的なものである。いずれも初診時年齢を重要視しているが，EORTC scoreは分化癌以外の甲状腺癌についても含めて検討している。

　甲状腺分化癌のように多数の因子が予後に関与する場合，これらを同時に考慮して関連度を推定し，偏りを是正する方法として，近年はCoxの重回帰型モデルによる予後因子の解析が行われている。呉ら^3）は対象を乳頭癌に限局し，1964〜1987年までの227例に対し検討を行い，診断時年齢が最も重要な予後因子であったとしている。年齢の因子に次いで，原発腫瘍の大きさ（T）と頸部リンパ節転移（N）の因子も予後に影響したという報告^4）や，遠隔転移の有無が予後に寄与していたという報告^5）もある。このように年齢を最重視する傾向があるが，リンパ節転移の程度も甲状腺癌の治療において無視できない問題であると思われる。

　当科における甲状腺悪性腫瘍の治療成績を報告すると同時に，分化癌における予後を左右する因子について検討を加えたので報告する。

To determine the appropriate surgical treatments of thyroid cancer, follow up results of 172 patients with malignant thyroid tumors were analyzed.

Ten-year total survival rate of papillary car-cinoma (n=138) was 92.2%, follicular carcinoma(n=11) was all alive.

The following facts were found to be unfavoura-bly related to the survival rate in differentiated thyroid carcinoma : pT4, pN1 and patients who need functional neck dissection. Age, sex and initial surgical method of the primary lesion were not significant.

From this study, we concluded that the thyroid gland can be preserved, but the patients with metas-tatic internal jugular nodes need the radical treat-ment.
