　はじめに

　Polymorphous low grade adenocarcinoma （以下，PLGAと略す）は，1980年代頃より盛んに臨床病理学的検討がなされており，1991年のWHOの新分類^1）に初めて記載された唾液腺腫瘍である。本腫瘍は，臨床的には遠隔転移がみられないなど予後が良好な点，病理組織学的にはその多彩な細胞配列と，均質で異型性に乏しい細胞構成という点が特徴的であり，もっぱら小唾液腺に発生するとされている。われわれが渉猟し得た限りでは，耳下腺に発生したPLGAは，これまでに4例しか報告がなく^2〜5），本邦ではWHOの新分類以降報告例がない。

　今回われわれは，PLGAに特徴的な病理組織像を示した耳下腺腫瘍の1例を経験したので，その臨床像ならびに病理組織像，および唾液腺腺癌について，文献的考察を加えて報告する。

We report a case of polymorphous low grade adenocarcinoma of the parotid gland. A 60-year-old male was admitted with a complaint of a lump in the right parotid region of one year duration. Clinical examinations, CT, MRI and Tc scintigraphy showed a tumor of benign or low grade malignancy in the right parotid gland. Superficial lobectomy of the parotid gland was performed and histopath-ological examination revealed polymorphous low grade adenocarcinoma.

Polymorphous low grade adenocarcinoma is an unusual tumor in the minor salivary gland, par-ticulary of the palate. To our knowledge, there were only five previous reports of this tumor in the major salivary glands. Clinicopathology of polymorphous low grade adenocarcinoma and adenocarcinona of major salivary gland was discussed.
