外鼻再建におけるBi-lobed flapの応用

Bilobed Flap for Nasal Reconstruction

　はじめに

　Bi-lobed flapは二葉の皮弁を作成し修復再建する方法であり，1918年Esser^1）が鼻尖部の再建に用いたのが最初とされている。その後身体のさまざまな部位での応用が報告されている。Zitelli^2），Elliott^3），McGregorら^4），Tardyら^5）は，外鼻の再建でその有用性が高いと報告している。外鼻皮膚欠損における再建での本術式の利点は，Tardyら^5）が述べているように色調に極めて優れていることと一期的手術が可能な点である。しかしながら，本邦での本術式の外鼻再建における有用性を報告したものは少ない。今回，著者はbi-lobed flapを用いた外鼻再建を3症例に行った。Bi-lobed flapの欠点であるdog ear （縫合端の余剰皮膚）の軽減のために茎を細くし，症例によっては部分的に皮下茎弁とすることにより，本術式の外鼻再建における有用性を確認したので報告する。

Bilobed flap design possesses excellent color match, primary closure of donor site, one stage repair, and minimal tissue distortion for nasal re-construction. The auther used Zitelli's design or McGregor's design for the bilobed flap. But, theirs design results in buckling and/or formation of small dog ears. Narrow base of the flap to prevent of buckling and dog ears would compromise its sur-vival. For improvement in design minimizes the problems of buckling and formation of dog ears, the author designed a partial subcutaneous pedicle flap. This improved design of the bilobed flap solved a difficult problem for buckling or dog ears.
