ワイドバンドティンパノメトリー（WBT）を用いた内リンパ水腫評価の予備的検討

The evaluation of endolymphatic hydrops by wideband tympanometry

はじめに

　ワイドバンドティンパノメトリー（wideband tympanometry：WBT）は，単一周波数（226Hz）で測定する従来のティンパノメトリーと比較して，226〜8000Hzの広帯域クリック音を使用してpeak pressureおよび大気圧（ambient）におけるアブソーバンスを測定する検査である^1,2）。WBTは外耳から発生する音響エネルギーと，鼓膜において反射される音響エネルギーを測定することで，中耳への音響エネルギーのアブソーバンスを測定する。本来は中耳の機能検査である。しかし，中耳機能にも影響を与える可能性がある内耳機能，特に内リンパ水腫の評価に応用可能であると考えられる。

　WBTの実際の測定結果として，はじめに健常者（図1）のデータを示す。周波数ごとのアブソーバンスと共振周波数が記録される。これまでWBTを用いた内リンパ水腫関連疾患の検討^3,4）では，小林ら^5,6）が著明前庭内リンパ水腫は低音域のアブソーバンスが上昇しており，正常範囲からの逸脱が大きかったと報告している。本検査法は簡便に行うことができるため，従来の内リンパ水腫推定検査^7,8）である蝸電図，グリセロールテスト，フロセミドテスト，フロセミド負荷前庭誘発筋電位（vestibular evoked myogenic potential：VEMP），ガドリニウム造影MRIなどと比較して短時間で行うことができる。そのため，内リンパ水腫のスクリーニング検査として応用できる可能性がある。また，内リンパ水腫に対する中耳加圧治療^9）などの治療の効果を，同一症例で評価するなどの目的で活用することも考えられる。

The aim of our study was to obtain wideband tympanometry findings in patients with defined Meniere's disease （MD）using MRI and in control patients. It also aimed to evaluate whether the data obtained had diagnostic significance. The ears with Meniere's were grouped as either the lesion side or the intact side of the MD. The absorbance values of different frequencies and resonance frequencies （RF） were compared between the lesion side of the MD, the intact side of the MD, and the control. The lesioned side of the MD showed a lower RF. Higher absorbance at lower frequencies and lower absorbance at higher frequencies were observed in patients with MD.
