血管塞栓術を要さず内視鏡的に摘出しえた鼻中隔glomangiopericytomaの1例

A case of endoscopically resected glomangiopericytoma of the nasal septum without the need for embolization

はじめに

　glomangiopericytomaは血管周囲筋様細胞に由来する低悪性度の軟部組織腫瘍で^1〜3），鼻腔腫瘍全体の0.5％以下を占める稀な腫瘍である^1）。1942年，Stoutら^4）により血管外皮細胞から発生する腫瘍としてhemangiopericytomaが報告されており，当初，hemangiopericytomaの亜型と考えられていたが，2005年のWHO分類改定によりglomangiopericytomaと定義された。治療は外科的切除が原則とされているが，血流豊富な腫瘍であり，出血を制御するために術前に血管塞栓術を行った報告もある^1）。

　今回われわれは，術前血管塞栓術を施行せずに内視鏡下鼻内手術で摘出した症例を経験したので報告する。

　Glomangiopericytomas are low-grade tumors that arise in the nasal cavity. There have been some reports on preoperative vascular embolization to control bleeding in tumors with increased blood flow. Herein, we describe a case in which the tumor was removed without preoperative embolization.

　The patient was an 81-year-old male. A red mass lesion with hemorrhage was found in the right nasal cavity. Based on the biopsy results, a vascular tumor was suspected. However, the endoscopic resection was performed without embolization. Based on the postoperative pathology, a diagnosis of glomangiopericytoma was made. Intraoperative bleeding was minimal, and there was no postoperative recurrence.
