The Surgical Treatment Strategy and Results of Parasellar Meningiomas in the Era of Radiosurgery

Ⅰ．はじめに

　傍鞍部髄膜腫に対する手術は，視神経等の脳神経の障害，穿通枝の損傷の危険性などを認め，なお手術侵襲に伴うリスクがある^19）．また近年，患者のquality of lifeを考える立場から，頭蓋底髄膜腫の治療においても，機能温存を目指した治療が求められている^15）．一方，低侵襲な治療であるra-diosurgeryによる髄膜腫に対する長期治療成績を含めた良好な治療結果が報告されてきている^12）．われわれは傍鞍部の髄膜腫に対して，機能温存に主眼をおいた．手術を行い，残存，再発腫瘍に対してはradiosurgeryを行うという方針で治療を行ってきたので，その治療成績について報告する．

We evaluated the surgical treatment results of parasellar meningiomas in the era of radiosurgery. We treated 24 patients of parasellar meningiomas surgically. The median age was 60 yrs (ranging from 29 to 82 yrs). The most com-mon tumor location was the sphenoid ridge in 12 patients and the tuberculum sellae in 7 patients. The pterional ap-proach using fronto-temporal craniotomy was performed for all patients. The residual or recurrent tumors were treated by gamma knife radiosurgery (GKS).
