急性咽喉頭炎様症状を主徴とした成人Still病の1例

A case of adult-onset Still's disease following acute pharyngolaryngitis symptoms

Ⅰ．はじめに

　咽頭痛と発熱を主徴とした症例に対して，耳鼻咽喉科医は，最初に急性咽喉頭炎や急性扁桃炎などを疑い膠原病を念頭に置くことはあまりない。今回，われわれは，急性咽喉頭炎のような咽頭痛と発熱を主訴として当科を受診した成人Still病症例を経験したので報告する。

　Adult-onset Still's disease（AOSD）is systemic juvenile arthritis which developed in adult patient. We reported a case of AOSD in a 52-year-old female with a chief complaint of fever and sore throat. Although she presented with severe symptoms,local inflammatory findings were not found. She was treated with antibiotics,but fever persisted. After investigation she was diagnosed as AOSD. Steroid therapy was effective. Therefore,patients with fever and sore throat in spite of no local abnormal findings should be considered to have AOSD. From the review of the literature,we suggested that sore throat of AOSD caused by cricoarytenoid arthritis.
