顎下腺膿瘍を生じた眼鏡の柄による咽頭外傷の1例

A case of submandibular abscess caused by penetrating injury to the oropharynx with temple of eyeglasses

Ⅰ　はじめに

　異物による口腔咽頭外傷はしばしば経験されるが，その症例のほとんどは幼小児である^1）。障害の程度は軽症がほとんどであるが，異物の種類や進入経路，深達度によっては深部の筋，血管，神経を損傷し，重篤となる可能性がある^1,2）。特に異物が既に除去された状態で受診する例では，十分な重症度の把握が困難であることも多く，受傷機転の詳細な問診，抜去された異物の状態の確認，口腔咽頭の注意深い診察を行い，患者の易感染性のリスクなどに十分留意して治療を行うことが重要である^3,4）。今回われわれは，眼鏡の柄による咽頭外傷で顎下腺膿瘍を生じ，外科的治療を要した成人症例を経験したので文献的考察を加えて報告する。

　A 61-year-old man with pharyngeal injury caused by the temple of eyeglasses. Four days later,penetrating injury to the oropharynx progressed to the submandibular space infection and abscess. He had history of diabetes mellitus（DM）and chronic renal failure. Surgical drainage and a submandibular gland dissection were performed. The etiology in this case was very rare. The early surgical treatment should be considered in patients,who do not response to antibiotics,with systemic disease and abscess of submandibular space.
