血腫除去を必要とした眼窩骨膜下血腫の1例

A case of subperiosteal orbital hematoma

Ⅰ　はじめに

　眼窩血腫は比較的稀な疾患であり，狭義の眼窩内血腫と骨膜下血腫に分類される。骨膜下血腫の原因としては外傷が多く特発性のものは稀である^1）。また，その経過は一般的に保存的療法で数週間から数か月で自然に吸収されることが多いとされるが^2），なかには視力障害をきたす症例もあり観血的な治療が必要となることもある。今回われわれは視力障害を伴ったため血腫除去術を必要とした原因不明の眼窩骨膜下血腫症例を経験したので，文献的考察を含め報告する。

　Subperiosteal orbital hematoma is rare,We report a case of right subperiosteal orbital hematoma in a 70-year-old woman who was presenting with sudden right superior eyelid swelling. She was found a spindle-shaped mass beneath the periosteum of the right orbit by CT,and subperiosteal orbital hematoma was diagnosed. Because of a profound visual loss,it was surgically drained by frontal approach 2 days after onset. The swelling and visual loss were improved.
