Ⅰ．はじめに

　悪性腫瘍において明らかな感染症や白血病の所見がないが，白血球が上昇する症例を認めることがある。その一因は腫瘍細胞が産生するgranulocyte-colony stimulating factor（以下，G-CSFと略す）と考えられている^1）。

　国内では，1977年にAsanoら^2）が肺癌のCSF産生腫瘍を報告して以来，肺癌や消化器癌で多数のG-CSF産生腫瘍が報告されているが，頭頸部癌の領域での症例報告は今回渉猟しえたかぎりでは自験例を含めて39例^3～20）あり，上顎洞癌の症例は自験例を含めて10例^5～11）しかなかった。今回，きわめて稀なG-CSF産生上顎洞癌を経験したので報告する。

　A 71-year-old woman presented with a right cheek swelling. CT scan examination revealed bone destruction of the right superior maxillary sinus wall and inferior orbital wall. The pathological diagnosis was poorly differentiated squamous cell carcinoma,based on analysis of biopsy from the canine fossa. Before treatment,number of the peripheral blood leukocytes and the level of serum G-CSF were extremely high at 24,900/μl and 359pg/ml respectively. After the treatment with chemoradiotherapy,the number of the peripheral blood leukocytes and the level of serum G-CSF were normalized. Furthermore,immunohistochemical analysis using anti G-CSF antibody showed positive staining on the tumor.
