　真菌性眼内炎が1年以上遷延したが，治療後の視力予後が良好であった1例を経験した。症例は74歳男性で，約1年前に悪性リンパ腫で腫瘍切除と化学療法を受け，寛解していた。司時期に右眼霧視を自覚，近医でぶどう膜炎を指摘され，ステロイド薬の全身ならびに局所投与を受けた。改善しないため当院眼科を紹介され入院となった。リンパ腫の再発を疑い，全身検索を行ったが診断に至らず，硝子体手術を施行した。切除硝子体の培養でCandida albicansが検出され，抗真菌薬を投与したところ著明な視力の改善をみた。ステロイド抵抗性のぶどう膜炎では，真菌性眼内炎も考えて経過に注意する必要がある。硝子体手術が診断，治療に有効であった。

A 74-year-old male was referred to us for blurring of vision in his right eye. Prior treatments with topical and systemic corticosteroid were futile. He had had malignant lymphoma which subsided by resection and chemotherapy one year before . The ocular findings were suggestive of recurrence of malignant lymphoma. After systemic examinations proved to be negative, his right eye was treated by vitrectomy. Culture of surgically obtained specimen showed Candida albicans. Additional treatment with antifungal agents induced improved visual acuity. This case illustrates that fungus endophthalmitis may be present in corticosteroid-resistant uveitis and that vitreous surgery may be of value in the diagnosis and treatment.
