穿孔性眼外傷に対する術後早期の副腎皮質ステロイド薬全身投与の検討

Evaluation of early systemic corticosteroid for penetrating ocular injury

　山形大学医学部附属病院眼科での最近5年間の穿孔性眼外傷の原因，治療および受傷早期の炎症による組織損傷抑制を目的とした副腎皮質ステロイド薬の投与について検刮した．症例は19例，19眼であった。性別は男性15例，女性4例。受傷原因は電動草刈り機によるものが3眼と最多であった。術直後から消炎目的に副腎皮質ステロイド薬の全身投与を施行されたものが14眼と多かったが，感染症などの合併症はみられなかった。副腎皮質ステロイド薬の投与によりフィブリンの消退は速やかであり，受傷早期の炎症による組織損傷の抑制に有効であると考えられた。

We reviewed 19 eyes of 19 cases of penetrating ocular injury during the past 5 years. The series comprised 15 males and 4 females. Injury by electric grass-mower was the most frequent cause of injury in 3 eyes. Fourteen cases received systemic corticosteroid immediately after injury in order to suppress inflammatory reaction. Fibirinous reactions decreased in these 14 cases more rapidly than in the other 4 cases. No intraocular infections developed in the steroid-treated cases. These cases illustrate that early treatment with systemic corticosteroid may be effective in selected cases of penetrating ocular injury.
