　症例は52歳の女性で，1989年，受診時に透明な水晶体の膨隆による閉塞隅角緑内障を認めた。Laser iridotomy．laser gonioplastyを施行したが眼圧は下降せず，さらにcore vitrectomyと隅角癒着解離術を施行したところ術後眼圧は大幅に変動した。1997年から水晶体の混濁が強くなったため，1998年2月に水晶体摘出と眼内レンズ挿入術を施行した。眼内レンズは嚢内固定されていたが，前方に位置し，前房は浅く，眼圧も変動を繰り返した。本症例における眼内レンズの前方移動は，水晶体赤道部の何か所かで周辺部の有形硝子体あるいは毛様体突起の過形成が関与している可能性があると推察した。

A 52-year-old woman was diagnosed as angle-closure glaucoma due to intumescent clear lens in her right eye. The elevated intraocular pressure (IOP) persisted after laser iridotomy and laser gonioplasty. Despite of additional core vitrectomy and goniosynechialysis resulted, IOP was fluctuated largely. The affected eye underwent lens extraction and intraocular lens (IOL) implantation for cataract 9 years after the initial diagnosis. The in-the-bag fixated IOL was positioned anteriorly, resulting in shallow anterior chamber, formation of peripheral anterior synechia (PAS) and in marked IOP fluctuation. The anterior shift of IOL may be causatively related either with the presence of anterior vitreous or hyperplasia of ciliary processes.
