漿液性網膜剥離のICG螢光造影所見

Indocyanine green angiography in serous retinal detachments

　網膜色素上皮の外血液網膜柵の破綻があり，続発性に漿液性網膜剥離が生じる疾患に対し，インドシアニングリーン（ICG）螢光造影を行った。対象は中心性漿液性網脈絡膜症31眼，胞状網膜剥離12眼，後極部剥離型の原田病14眼，妊娠中毒症2眼である。フルオレセイン螢光造影で見られる色素上皮からの漏出や網膜下腔の色素貯溜などは，ICG螢光造影でも類似した所見を示したが，フルオレセインに比べて検出しにくかった。これら網膜下のICGからの螢光を脈絡膜のそれと区別する必要があり，フルオレセイン螢光造影との比較が重要となる。4疾患に共通する脈絡膜のICG螢光造影所見として，血管透過性亢進があった。妊娠中毒症と原田病では，経時的な観察で拡大する過螢光像や血管の輪郭の不明瞭化が観察された。また，中心性漿液性網脈絡膜症と胞状網膜剥離では，過螢光を示す脈絡膜の組織染があった。中心性漿液性網脈絡膜症，胞状網膜剥離，原田病などでは，色素上皮レベルでの透過性亢進の基礎病変として，脈絡膜血管の透過性亢進があることが結論される。

Indocyanine green (ICG) angiography was performed in 59 eyes with various types of serous retinal detachment. The series comprised central serous chorioretinopathy 31 eyes, bullous retinal detach-ment 12, Harada's disease 14 and toxemia of pregnancy 2. All the eyes showed dye leakage through the retinal pigment epithelium by fluorescein angiography. ICG angiography showed essentially similar findings regarding dye leakage and pooling as seen by fluorescein. As a unique feature, ICG angiography showed leakage from choroidal vessels in Harada's disease and toxemia of pregnancy. Also, choroidal tissue staining by ICG was present in central serous chorioretinopathy and bullous retinal detachment. The findings seemed to suggest that choroidal vascular hyperpermeability plays a causative role in damage to retinal pigment epithelium with resultant accumulation of subretinal fluid.
