非ステロイド療法時の円板状角膜炎の経過と病型の推移

Clinical course of disciform keratitis treated without corticosteroid

　実質型角膜ヘルペスでは，角膜瘢痕を生じたり，角膜の菲薄化や角膜穿孔にいたる重篤な症例を経験することも稀ではない。筆者らはこれまで実質型角膜ヘルペスをステロイドをまったく使用することなく治療を行ってきたが，その際，著しい視力低下や炎症の遷延化，角膜穿孔などを経験することはなかった。今回これを確認する目的で過去10年間に筆者らが経験した症例についてretrospectiveに検討を行ってみた。当科での治療以前にもステロイド使用の既往のない43症例を対象とした。治癒の判定は角膜浮腫，角膜浸潤，虹彩炎の消失をもって行ったが，全例とも平均2か月で治癒可能であった。治癒時の角膜混濁は軽微なことが多く，視力は不変の2例を除き全例で改善していた。初診時より治癒までの角膜所見の中に，従来より実質型角膜ヘルペスの難治な病型とされている水疱性角膜症，壊死性角膜炎，デスメ瘤，角膜穿孔はなく，また，上皮型角膜炎，前房蓄膿，角膜実質内新生血管も観察されなかった。今回得られた臨床像は，ステロイドにより修飾されない実質型角膜ヘルペスのオリジナルな病像を示しているものと考えられた。

We evaluated the clinical course of 43 eyes with disciform keratitis. All the cases were mainly treat-ed with antiviral agents and topical atropine. No case received corticosteroid, either topically or systemically. It took an average of 65 days for disappearance of corneal edema and iritis. At the time of healing, corneal opacity was absent to mild in the majority of cases. Visual acuity improved inall but 2 eyes, There was no instance of epithelial keratitis, hypopyon, bullous keratopathy, necrotiz-ing keratitis, descemetocele, corneal performation or stromal neovascularization. The observed fea-tures in the present series seemed to be characteris-tic for herpetic stromal keratitis unmodified by corticosteroid.
