糖尿病患者の両眼性一過性乳頭浮腫の1例 （diabetic papillopathy）

Transient bilateral edema of the optic in a diabetic subject

　43歳の女性の糖尿病患者にみられた両眼性一過性乳頭浮腫の臨床経過について報告した。

　両眼底共背景糖尿病性網膜症がみられ，最初左眼乳頭浮腫，次いで9日後に右眼乳頭浮腫がみられた。左眼乳頭浮腫は発症より9日目，右眼乳頭浮腫は22日目より軽快しはじめ後に軽度の乳頭萎縮を残した。検眼鏡的に乳頭は充血腫脹し，螢光眼底造影においても乳頭部血管拡張および色素漏出がみられ，filling defectの所見はなかった。

　全経過中，糖尿病のコントロールは良好で，CT scan瞳孔反応，luminance VEP等神経学的検査は正常で，視力は両眼共0.5前後に低下した。視野検査では両眼共マリオット盲点の拡大と下鼻側視野異常を認めた。

　両眼乳頭浮腫の消退と共に，視力は右0.9，左0.7に改善し，右視野は正常となり，左視野も軽度改善を認めた。

　本症例の臨床像は，Appenらのいうdiabetic papillopathyに相当すると考えられた。

Transient edema of the optic disc developed bilaterally in a 43-year-old female with diabetesmellitus which was of short duration and well-controlled. Mild background retinopathy was present in both eyes. Edema of the optic disc appeared in the left eye, to be followed by a similar feature in the right eye 9 days later. The optic nerve head was hyperemic with dilatation of superficial capillaries. There was no filling defect of dye in the disc region by fluorescein angiography.
