Abstract Purpose:In cataract surgery, continuous curvilinear capsulorhexis is an important in the subsequent surgical operation. Cataract surgery in the presence of corneal opacity requires devices for enhanced visibility. We report two cases of cataract surgery with severe corneal opacity using precision pulse capsulotomy(PPC).

Cases:Case 1 was a 78-year-old female patient. She had a corneal opacity due to limbal stem cell deficiency in her left eye(LE). Preoperatively, the best corrected visual acuity(BCVA)in the LE was 2/100, the intraocular pressure(IOP)was 17 mmHg and the anterior chamber(AC)depth was 2.5 mm. After 1% sodium hyaluronate(OPEGAN Hi® 0.85)was injected to fill the AC, the tip of the PPC device(ZEPTO® System, Mynosis, USA)was inserted to the AC and positioned on the center of the anterior capsule. The capsulotomy was performed by discharging electrical energy from the ring of the tip. The estimated diameter of the circle of capsulotomy was 5.2 mm. The remaining surgery was conducted with the phaco-emulsification technique using chandelier retro illumination. No capsule tear or complications occurred during this surgery. Case 2 was a 64-year-old male patient. He had severe corneal opacity due to granular corneal dystrophy. Preoperatively, the BCVA in the right eye was 4/100 and the AC depth was 2.8 mm. As in case 1, anterior capsulotomy was performed using ZEPTO® PPC device. There were no intraoperative complications.

Result:The ZEPTO® PPC device facilitated the creation of a precise capsulotomy in the challenging cataract surgery cases with corneal opacity. The use of this device not only reduces the risk of intraoperative complication, but also reduces the surgical stress.
