要約　目的：毛様筋ストレッチ（CMS）による小学生の近視予防効果の有無を検討すること。

対象と方法：CMSは，近見と遠見を交互に繰り返すことで毛様筋に緊張と弛緩を繰り返させる方法である。約1秒間隔で5回繰り返し，所要時間は10秒で特別な道具は必要としない。この方法で近業後の毛様筋の「過緊張状態」を改善し，近視を予防できると考えた。東京都内の小学校（A校）で2001〜2018年にわたり年1回CMSを説明し，近業後に1回行うように勧めた。一方，A校から1.5kmの距離にある小学校（B校）ではその間CMSについては何も説明を受けていなかった。A校とB校の2001〜2018年の裸眼視力の変化，および2018年時点の両校の屈折値（SE）を比較検討した。

結果：裸眼視力1.0未満児童割合はA校では51.7％（2001年）から27.3％（2018年）に低下し，B校では31.4％（2003年）から44.5％（2018年）に増加した。

　2018年時点のSE≦−0.5D割合はA校では25.8％，B校では35.3％であった。学年ごとの平均SEでは，3年生（p＜0.005）と6年生（p＜0.01）で両校間に統計的有意差がみられた。全学年の平均SEでも有意差（p＜0.05）を認めた。

結論：CMSには小学生の近視を予防する効果があると考えられた。

Abstract　Purpose：To evaluate the effectiveness of ciliary muscle stretching（CMS）for preventing myopia in primary school children.

Subjects and methods：CMS is a technique for relaxing and straining ciliary muscles through near and distant gazing, alternatively. Gazing is repeated five times at 1-second intervals for 10 seconds；no special equipment is necessary. CMS was expected to prevent myopia by restoring overstrained ciliary muscle to its original state after near work. CMS was introduced annually at a primary school in Tokyo（school A）from 2001 to 2018；it was recommended for use after near work for the prevention of myopia. At another primary school（school B, 1.5 km away from school A）, CMS was not used during this period. Uncorrected visual acuity（UCVA）scores from 2001 to 2018 and refractive errors — spherical equivalent（SE）— in 2018 were compared between schools A and B.

Results：The proportions of UCVA＜1.0 in school A decreased from 51.7％ in 2001 to 27.3％ in 2018；in school B, it increased from 31.4％ in 2003 to 44.5％ in 2018. The proportions of SE≦−0.5D in 2018 was 25.8％ in school A and 35.3％ in school B. Significant differences were found in mean SE between the two schools in the third（p＜0.005）, sixth（p＜0.01）, and all（p＜0.05）grades.

Conclusion：CMS was effective at preventing myopia in primary school children.
