インターフェロン（IFN）治療における血球動態とIFN網膜症についての検討

要約　目的：インターフェロン（IFN）網膜症と血球動態との関連の報告。

対象と方法：過去4年間にB型またはC型の慢性肝炎に対してIFN投与を受けた149例を対象とした。男性66例，女性83例で，年齢は平均65歳である。治療中に網膜症が発症した66例と，発症しなかった83例につき，血液学的所見を検索した。

結果：血液の白血球，赤血球，血小板は，いずれもIFN投与後に減少し，汎血球減少を示した。白血球については，網膜症発症群と非発症群とで有意差はなく，ヘモグロビンと血小板については，網膜症発症群で顕著であり，有意差があった。治療開始後のヘモグロビン値が5g/dl以下に低下した群では有意に網膜症が多く，血小板の減少が1万/dl未満の症例では網膜症が有意に少なかった。

結論：IFN網膜症の患者では，治療開始後のヘモグロビン値と血小板の大きな減少があった。

Abstract　Purpose: To report the correlation of interferon retinopathy and hematological findings.

Cases and Method: This retrospective study was made on 149 patients who received interferon therapy for chronic hepatitis B or C during the past 4 years. The series comprised 66 males and 83 females. The age averaged 65 years.

Results: Interferon retinopathy developed in 66 cases（44.3％）. The 149 cases showed decrease in leukocytes, erythrocytes, and platelets after start of interferon therapy when seen as average. There was no difference in leukocyte reduction between 66 cases who developed retinopathy and the other 83 cases. Decrease in hemoglobin and platelets was more marked in cases who developed retinopathy. Retinopathy was more frequent in cases who showed reduction of hemoglobin less than 5 g/dl. Retinopathy was less frequent in cases reduction of platelets by 10,000/μl after start of treatment.

Conclusion: Cases who developed interferon retinopathy showed marked reduction in hemoglobin and plates.
