眼内金属片による角膜鉄錆症の術前後の評価にScheimpflug画像が有用であった1例

要約　目的：眼内の金属片の摘出術を行った角膜鉄錆症で角膜混濁の定量的評価に用いるScheimpflug画像のdensity解析（以下，SD）が術前後の評価に有用であった1例を報告する。

対象と方法：57歳，男性。2012年，右眼に金属片による角膜裂傷に対して他院で角膜縫合を受けた。2014年9月より右眼痛を自覚し，9月下旬に和歌山県立医科大学眼科を受診した。角膜赤褐色混濁と眼窩CTで眼内金属片滞留を認め，眼球鉄錆症，角膜鉄錆症と診断し，金属片の摘出術を行い，術前後の角膜混濁の評価にSDを用いた。

結果：術前角膜のSDは右眼94.4であったが，術後角膜混濁は改善し術後6か月後に43.1に改善した。

結論：眼内の金属片を摘出することで角膜鉄錆症とSDは改善し，角膜混濁の評価にSDは有用であった。

Abstract　Object: We reported a case that density analysis of Scheimpflug image（SD）for quantitative evaluation in a corneal siderosis patient was useful to evaluate before and after surgery that removed the metal pieces in the eye.

Methods: 57-year-old man. In 2012, he was gone to the surgery for corneal laceration in the right eye by the metal pieces in another hospital. He felt eye pain in September 2014 and he admitted Wakayama Medical University hospital on September 24. We diagnosed ocular siderosis and corneal siderosis because we admitted cornea reddish brown opacity and intraocular metal piece residence by orbital CT, and we removed. We used SD for evaluation of corneal opacity before and after surgery.

Results: Although the SD of cornea in the right eye was 94.4 before surgery, the SD was improved to 43.1 after surgery 6 months as well as improvement of corneal opacity.
