瞳孔異常を伴わない動眼神経麻痺の1症例

Oculomotor nerve paralysis with sparing of the pupil

緒　言

　一般に，瞳孔異常を伴わない動眼神経麻痺は，糖尿病に合併することが特徴とされている。しかし，今回われわれは，糖尿病のない男性に，瞳孔異常を伴わない動眼神経麻痺が発症し，脳血管撮影によつて，脳底動脈のelongationがみられ，しかも糖尿病に合併する場合と同様，およそ3カ月間で治癒した症例を経論した。わが国では，この様な症例の報告はないので，ここに経過を報告したい。

A 74-year-old male developed left-side oculo-motor nerve palsy with sparing of the pupil. There was mild pain located around the affected eye. Cerebral angiography showed severe tortuo-sity of the basilar artery and obstruction between the left vertebral and basilar artery. There was mild hypertension. Diabetes mellitus was routed out through systemic examinations. The oculo-motor nerve palsy came to a spontaneous cure three months after the onset.

The present case is of interest as oculomotor nerve palsy with sparing of the pupil is usually peculiar to diabetes mellitus.
