興味ある多視を訴える1症例

A case of hysterical polyopia after squint operation

緒　言

　近年眼科領域において心因性の視機能障害が注目され，眼心身症，眼ヒステリーとして興味ある症例が報告されている。殊に小児の眼科診療に際しては心因性の視機能障害に注意をはらう必要があると思われる。一方「眼心身症」という言葉が広義に解釈され，心因性の視機能障害を総称して用いられているようであり，眼心身症と眼ヒステリーの異同がいささか混乱している。

　私たちは斜視手術後に三重視を訴える症例を経験し，精神科医による診察の結果ヒステリーの機制によることが推察された。このような症例は他になく大変興味ある症例と考えるので報告した。

An 8-year-old boy complained polyopia alter squint operation. He was noted to be esotropic as he was one year old. At the age of six he underwent the right eye operation, supposedly recession of medial rectus muscle. After the ope-ration a transient diplopia developed for a few clays. One month later, he underwent the left eye operation. After the operation he complai-ned persistent binocular diplopialn a few months interval, a third and fourth operation was done in the left eye and the right eye successively. After the fourth operation he developed mono-cular diplopia and binocular triplopia.
