前交通動脈瘤による片眼失明の1例

Unilateral blindness due to anterior communicating aneurysm. A case report

緒　言

　脳底部に発生する脳動脈瘤により，さまざまな視野障害が起ることが報告されている。内頸動脈瘤，特に巨大動脈瘤によつて視神経が障害される場合には，しばしば動眼神経その他の脳神経の症状を伴うため，診断は比較的容易であると考えられる。中大脳動脈瘤による視野欠損は，視神経に対する直接の障害によるというよりは，脳内での視放線の障害によるものが多いと考えられる。前交通動脈瘤は，その解剖学的な関係より，視神経に直接に障害を及ぼすことがあるものと考えられるが，その頻度はかなり稀なものである。われわれは，急速に片眼の失明をきたし，CT，頸動脈写および手術によつて，前交通動脈瘤による視神経の直接の圧迫による障害と診断された1例を経験したので報告する。

A 54-year-old man had an episode of subara-chnoid hemorrhage four years previously. Two years later, he noticed transient haziness in his left eye which persisted for about 30 minutes. On the following day, an upper visual field de-fect of his left eye appeared following severe left occipital headache, and progressed to the entire visual field in 10 minutes, as if a curtain were descending without any disturbance of conscio-usness. Neurological examinations revealed blind-ness, optic atrophy and absent light reflex in his left eye. Left carotid arteriogram at that time was negative except for signs of ventricular di-latation.
