緒言

　本邦において井街^1）が昭和15年（1940）に視力，視野，髄液圧の動揺する球後視神経炎症例に，視交叉部クモ膜炎の存在を想定し，これに対し積極的に治療を加える目的で開頭により視交叉部露出を行ない，良い成績を得た^2）。以来現在に至るまで同疾患に対しての開頭手術療法についての検討が行なわれ^3），その有効性は確立したと一般に認められるに至つた。

　一方欧米においては，Cushing^4）以来視交叉部クモ膜炎の存在，およびこれに対する開頭術の有効性をのべる論文が多数見受けられる^5）6）にもかかわらず，なお一般的でない印象を受ける。

Long-term follw-up findings are presented on two caucasian cases of arachnoiditis optico-chiasmatica treated by craniotomy. While the final visual outcome was favorable, the author noted fluctuations of cerebrospinal fluid pres-sure, visual acuity and field during the post-operative period. Analysing those results, it is suggested that the fluctuations of visual acuity and field seems to show the possibility of reco-very of them. And also the importancy of con-troling of the circulation of S. C. F. for recovery of visual function was discussed.
