水晶体前嚢剥離を伴つた硝子工白内障—臨床的および電顕的検索

Glassblower's Cataract, Accompanied by True Exfoliation of the Lens Capsule:Clinical and Electron Microscopic Study

緒言

　硝子工なとの高熱下労働者にみられる火熱白内障に，しばしば水晶体前嚢の剥離を伴うことは，1922年El3chnig^1）が最初に報告し，Vogt^2）は，水晶体前面瞳孔領で水晶体嚢の表層が薄く剥れるのを見て，Feuerlamelle （火薄板）またはSolutiolaminae superficialis （表層薄板剥離）と命名した。その後HollowayとCowan^3）による報告などが相次いだが，一方これらの人々は，嚢性緑内障に見られる水晶体前嚢の落屑をも同一のものと考えていた、ところが，1954年Theobald^4）水晶体前嚢の剥離を，その臨床像と病理組織像とから詳細に検討した結果，火熱白内障に伴う前嚢剥離と，嚢性緑内障に見られる水晶体前嚢落屑とは，全く異なるものであることを明らかにし，前者をtrue exfoliation，後者をpseudoexfoliationと名づけた。この論文をきつかけとして，欧米ではpseudoexfoliationにっいての研究が進んだが，true exfoliationの報告は激減した。その原因は，工場の急激な近代化に伴つて，眼の防護手段が強化されたためであろう。

　ところで，本邦におけるtrue exfoliationについての報告は，1942年の池田・山田　^5），1943年の池田・山田・岡崎^6）によるものが認められるのみである。

Detatchmcnt of the anterior lens capsule in a glassblower, aged 73, was studied clinically and electron microscopically.

Histologically this case showed a rent in the total layers of the lens capsule. The location of this rent was correlated with the areas clinically noted to have a thin, homogeneous, transparent membrane continuous with the capsule. The superficial layer was adherent to the exfoliated capsule but missing in the unaffected part of the anterior capsule. The underlying epithelial cells were degenerated and their atrophy was remarkable in the splitting area.
