SMONの眼科臨床的ならびに病理学的検討

Ophthalmic Findings and Pathohistology of the Optic Nerve in SMON (Subacute Myelo-Optico-Neuropathy)

I．緒言

　いわゆる腹部症状をともなう脳脊髄炎症は，清野ら^1）高崎ら^2）によって報告された症例に始まり，1964年日本内科学会のシンポジウム^3）にとりあげられるに至つた。本症の名称については，さまざまの命名があつたが，現在では椿ら^4）のいうSMON （Subacute myelo-optico—neuropathy）が，一般的となつている。本症の原因については，感染説，ビタミン欠乏，代謝障害説，薬物中毒説など，いろいろ考えられているがいまだに決定的でない。感染説については新宮ら^5）6）のECHO−21型ウイルス説もあるが，これは追試による確認がなされておらず甲野^7）のslow virus感染説などもあるが，まだ明確でない。

　SMONの眼症状に関する報告は，眼科領域でもかなり多数みられるようになつたが，今回われわれは岡山県下におけるSMON多発地である井原地区および湯原地区の眼科的集団検診の結果およびSMONで死亡した患者の視神経を病理組織学的に検索したので報告する。

1. One hundred and twenty seven patients with established diagnosis of SMON (SubacuteMyelo-Optico-Neuropathy) from Okayama-ken have been examined opthalmologically. Thirty two cases had ocular manifestation such as bi-lateral visual disturbance in all cases. Findings of the fundus in these cases were as follows: Twenty two cases manifested simple atrophy of the optic nerve, eight cases temporal pallor of the disc, and two cases neuritis optica.

2. The optic nerves of three patients clinically diagnoed as SMON were examined pathohisto-logically. The results obtained were as follows:
