I．緒言

　胎生初期に内頸動脈と脳底動脈との間には，いくつかの吻合路があるが，発育の途上で次第に吸収されて消退するのが普通である。しかしまれに，これらの吻合路が成人で遺残していることがある。この遺残吻合路は上位からpersistenttrigeminal artery, persistent acoustic　（otic）arteryおよびpersistent hypoglossal arteryの3種が報告されているが，persistent trigeminalarteryは最も多く，100例以上をかぞえ，またpersistent acoustic arteryはきわめてまれで，これまで4例にすぎない。またpersistent hypo—glossal arteryもかなりまれであつて，これまで26例の報告があるのみである。著者らは脈なし病に合併したpersistent hypoglossal arteryの興味ある1例を経験したので報告する。

A 22-year-old female presented typical fin-dings of pulseless disease. Cardinal subjective findings consisted of bilateral hypoacusia forthe last 7 years, headache and attacks of syn-cope by upward movement of the head since 3 years before. Ophthalmological studies revealed decreased pressure in the central retinal artery, dilatation of retinal veins, formation of capillary microaneurysms and arteriovenous anastomoses in the fundus periphery. The blood pressure in the upper extremities wers too low to be mea-sured.

Angiography proved the both subclavian arte-ries to have completely occluded.
