I．はじめに

　胎生期における4本の内頸動脈脳底動脈吻合血管（carotid-basilar anastomosis）の遺残動脈の1つであるpersistent primitive hypoglossal artery（以下PHAと略す）は，1889年，Batujeff^1）により報告されて以来，脳血管撮影の普及により，近年その報告例が増加している．遺残動脈が臨床上問題とされるのは，血管奇形，特に脳動脈瘤を合併する確率が高いと論じられるためである．最近著者らは，右PHA自体に形成された動脈瘤を経験したので，若干の文献的考察を加えて報告する．

Abstract

The authors describe a case of persistent primitive hypoglossal artery aneurysm in a 42-year-old woman who had complained of headache, mainly in the occi-put, for 5 days prior to admission. Because of a sudden exacerbation of the headache associated with vomiting, she was hospitalized on July 31, 1988. on admission, a cranial computed tomography scan demonstrated a high density lesion in the basal cisterns which sug-gested subarachnoid hemorrhage (SAH).
