I．まえがき

　近年精神安定剤としてのChlorpromazineの優秀性が確認されてから，ChlorpromazineのようにParkinson症候群，肝機能障害その他の副作用を起さずに，しかもこれと同等あるいはそれ以上の偉力をもつPhenothiazine誘導体の探求が続けられた。1954年，Chlorpromazineの構成分の一部をN-methyl piperidine環でおき換えたProchlorperazine　（2—chloro−10—〔2—N—（methyl—piperidyl）—ethyl〕phenothiazine）がSandozで合成され，NP207の名称で試供されたが（第1図），間もなくKinross-Wright, Verry, Rintelnら，GoarらおよびBurianらによつて，NP207はしばしば網膜色素変性を起すことが相次いで報告されたために，ついに市場に現われるに至らなかつた。その後NP207のClをS-CHでおき換えたThioridazine （2—methylmercapto−10—〔2—（N—methyl−2—piperidyl）—ethyl〕—phenothiazine）が合成されてから（第1図），副作用の少ない新らしい優れた精神安定剤として内外ともにこれが広く用いられ，現在に至つている。

A female, aged 37 years, having suffered from schizophrenia since 1953, had been given large doses of tranquilizer. On July 7. 1964, she was started on thioridazine (Melleril orally given in a daily dose ranging 600~1200 mg (average 1046 mg) for 74 days, the total amounting to 77.4gm. On September 19 that is, after the end of this period, she began to complain of blurred vision and night blindness.
