磁気共鳴画像を用いた網膜剝離の診断

Diagnosis of retinal detachment by magnetic resonance imaging

要約　目的：磁気共鳴画像検査（MRI）により，シリコーンオイル注入眼で網膜剝離と関連する所見を検索した報告。方法：MRIでは表面コイルとハーフフーリエ法で撮像し，撮影時間を短縮すると同時に高い解像度を得た。結果：網膜剝離手術の既往がある水疱性角膜症の1例ではMRIで網膜剝離がないことが判明した。黄斑円孔手術を行った1眼では，仰臥位でシリコーンオイルが前方に移動する所見がMRIで得られ，網膜剝離が確認された。緑内障インプラントの既往がある1眼では，シリコーンオイルがインプラント経由で眼外に流出する所見が得られたが，網膜剝離は復位していた。結論：表面コイルとハーフフーリエ法を用いるMRIは，視認が困難なシリコーンオイル充填眼の眼底状態の評価に有用であった。

Abstract.　Purpose：To report findings by magnetic resonance imaging（MRI）in eyes injected with silicone oil in the vitreous. Method：Three eyes were examined by MRI using a microscopy surface coil and half-Fourier single-shot fast spin echo（SSFSE）that improves resolution and shortens acquisition time. Results：MRI showed absence of retinal detachment in an eye with bullous keratopathy and past history of retinal detachment. MRI showed anterior displacement of silicone oil in supine position in another eye after macular hole surgery. The findings led to detection of retinal detachment. In a third eye implanted with glaucoma valve,MRI showed leakage of silicone oil out of the eye through the valve but no retinal detachment. Conclusion：MRI with microscopy surface coil and SSFSE was useful in evaluating the intraocular condition in eyes injected with silicone oil in the vitreous.
