要約　目的：特発性黄斑円孔に対する硝子体手術の成績と，術後のうつ伏せ期間との関連の報告。対象と方法：過去27か月間に2つの医療施設で硝子体手術を行い，3か月以上の経過を追えた特発性黄斑円孔54例55眼を診療録に基づいて検索した。全例で内境界膜剝離を行い，SF₆によるタンポナーデを実施した。術後のうつ伏せ期間は31眼が数日間，24眼が数時間であった。結果：術翌日の眼圧，初回閉鎖率，最終閉鎖率は両群間に有意差がなかった。術後合併症として，数時間群の2眼に眼内レンズの虹彩捕獲が生じた。結論：特発性黄斑円孔に対する硝子体手術では，数時間のみの術後うつ伏せ姿勢でも良好な成績が得られる。眼内レンズの虹彩捕獲には留意する必要がある。

Abstract.　Purpose：To report the outcome of surgery for idiopathic macular hole with different durations of face-down positioning. Cases and Method：This retrospective study was made on 55 eyes of 54 patients who received surgery for idiopathic macular hole in two institutions during the past 27 months. All the eyes underwent peeling of internal limiting membrane and tamponade with sulfur fluoride. Following surgery,31 eyes remained in face-down position for a few hours and 24 eyes for a few days. Results：There was no difference between the two groups regarding intraocular pressure on the day after surgery,initial and final rate of hole closure. Iris capture of intraocular lens occurred in 2 eyes that remained in face-down position for a few hours. Conclusion：There was no difference in the rate of success after surgery in two groups that remained in face-down position for a few hours or a few days. Iris capture of intraocular lens is a potential complication after surgery.
