Abstract Purpose:To report the outcome of surgery for macular hole using inverted flap of internal limiting membrane(ILM)in different directions.

Cases and Method:This retrospective study was made on 27 eyes of 27 patients who received macular hole surgery inverted ILM flap technique during the past 39 months. The series comprised 16 males and 11 females. The inverted flap was classified into superior and other directions.

Results:Six months after initial surgery, macular hole closed in 21 out of 27 eyes(78%). Macular hole closed in 25 out of 27 eyes(93%)after single or multiple surgeries. Use of viscoelastic substances on the inverted flap significantly improved the surgical outcome(p=0.027). Out of 6 eyes that failed to respond to initial surgery, macular hole closed in 2 eyes by revision of ILM flap, in one eye by enlarging the ILM peeling, and in one eye by ILM transplantation. Visual acuity after 6 months of surgery was significantly correlated with that before surgery(p=0.028). Visual acuity after 3 months of surgery was significantly better when the ILM flap was made in superior direction(p=0.003). Direction of ILM flap was correlated neither with rate of hole closure, foveal thickness or visual acuity after 6 months of surgery.

Conclusion:Surgery with inverted ILM technique showed a high rate of macular hole closure.
