大学病院でのヤギー文書・画像による眼科診療管理と専用電子カルテの構築

Electronic medical records and management of patients for ophthalmology in a university hospital using Yahgee Document/View

要約　背景：大病院では，内科系を中心に電子カルテが構築され，患者の動線が複雑な眼科は対応に苦慮する。目的：全診療科用の電子カルテに，文書と画像管理を主目的とするサーバを全科用に追加導入した診療システムと専用カルテの報告。対象と方法：岡山大学病院眼科に導入した新システムを1か月間の新患103人，再診2,396人に対して使用し，評価した。結果：デュアルモニタを設置し，左に富士通カルテ，右にヤギーカルテを表示した。視力，屈折，眼圧は自動的に取り込んだ。眼科カルテには初診版，再診版，入院版を用意し，眼底写真，OCT，細隙灯写真などの画像を貼り付ける枠を設けた。再診予約，処方，検査所見，会計処理は富士通画面で行った。結論：電子カルテ本体と並列して眼科外来での診療進捗状況を画面に表示し，視力などの検査結果を眼科カルテに自動的に取り込むことで，多人数の外来診療が可能になった。

Abstract.　Background：Electronic medical records may not be ideal for ophthalmology due to specialized examinations and findings. Purpose：To report a new system for ophthalmology added to Fujitsu electronic medical records system at our university hospital. Method：This system was used to handle outpatients in ophthalmology for one month. New and follow-up patients totalled 103 and 2,396,respectively. Results：Basic data were presented on the left hand and ophthalmologic ones on the right hand of dual monitors. Visual acuity,refraction and intraocular pressure were automatically transferred into the system. Spaces were provided for slit-lamp or fundus photographs. Prescriptions,billing and next appointments were handled by the Fujitsu system. Conclusion：The new system facilitated handling of a large number of patients in ophthalmology.
