Retinal migraineと考えられる若年性再発性網膜動脈分枝閉塞症の1例

A case of recurrent juvenile branch retinal artery occlusion suggestive of retinal migraine

　39歳女性が左眼の上方視野欠損を突発して受診した。15年前に僧帽弁閉鎖不全症で人工弁置換術を受けて以後，抗凝固薬などを内服している。同じく，15年前から視野障害の突発が右眼または左眼に合計4回あり，他院で網膜動脈分枝閉塞症と診断されている。矯正視力は左右眼とも1.5であった。左眼の下耳側動脈の支配領域に蒼白化と浮腫があり，網膜動脈分枝閉塞症と診断した。発症から10時間後に行った蛍光眼底造影では，左眼眼底に循環障害はなかった。過去15年間に繰り返している網膜動脈分枝閉塞症が血栓による可能性は小さいと判断した。15年前の人工弁置換術以来，視覚異常を伴う片頭痛が頻発し，その発症時にはストレスが多く，興奮状態にあった。これらの状況から，自立神経失調や過呼吸に伴う網膜動脈の攣縮，いわゆるretinal migraineの発作があり，それが強いときに網膜動脈分枝閉塞症が発症したと推測した。

　A 39-year-old woman presented with sudden visual field defect in her left eye. She had received artificial valve due to mitral valve insufficiency 15 years before. She had been under anticoagulants ever since. A total of 4 attacks of visual field defect occurred in the right or left eye during the past 15 years. The attack had been diagnosed with branch retinal artery occlusion（BRAO）. When seen by us,her visual acuity was 1.5 in either eye. In her left eye,the retinal area irrigated by inferior temporal artery had become opaque and edematous. The finding was compatible with the diagnosis of BRAO. Pathological circulation was ruled out in her left eye by fluorescein angiography performed 10 hours after the attack. She had had recurrent attacks of migraine with visual disorders during the past 15 years. The observed BRAO appeared to be secondary to spasm of retinal arteries,or so-called retinal migraine.
