　膝前十字靱帯(ACL)修復術後において，再度外傷がない限り，膝関節血腫が出現することは少ない．症例は41歳の女性で膝前十字靱帯再建術後16年経過した後から，何ら誘因なく膝関節血症が出現した．その原因は，ACL再建に使用したpull-outされていたナイロン糸の幾重にも結紮していた先端の鋭になった部分が，上下に動くことによる滑膜への刺激であった．関節内に鋭的な異物が突出する場合は，術後に長期間経過した後であっても膝関節血症を呈することがあるため，手術の際は関節内に鋭的な異物を突出させないようにする必要がある．

　Recurrent hemarthrosis of the knee joint in the absence of any clear cause is diagnosed as hemarthrosis of the knee joint.　We report a rare case in which a hematoma of the knee developed in the absence of any history of trauma or any other obvious cause 16 years after anterior cruciate ligament repair.　The hemarthrosis of the knee was caused by irritation of the synovial membrane by a sharp region at the tip of an extruded nylon thread that had been ligated several times.　It is necessary to be very careful to ensure that no protruding sharp foreign objects are left behind within the joint.
