　Sprengel変形に対しGreen変法を施行した1例を経験したので報告する．症例は生後8カ月の男児で，左肩甲骨高位を指摘され当科へ紹介となった．単純X線写真でも左肩甲骨の上方偏位を認めた．われわれはSprengel変形の手術の適応年齢とされる3歳まで経過観察し手術を行った．術前の左肩関節外転角度は90°，肩甲骨上角高位の左右差は5椎体であったのに対し，術後は肩甲骨上角高位の左右差は3椎体と改善した．術後3年8カ月の現在では外転角度は150°と改善し，過去の報告と比較してもほぼ同等の成績が得られた．

　We report a case of Sprengel's deformity treated by a modified Green procedure.　We examined an 8-month-old boy with an elevated left scapula, and performed a modified Green procedure when he was 3 years old.　Preoperatively, abduction of the left shoulder was 90 degrees, and the height of the left scapula on plain radiographs was at C5.　Postoperatively, the height of the left scapula on plain radiographs was at C7.　At the 3.8-year follow-up examination, abduction of the left shoulder had improved to 150 degrees, and the height of the left scapula on plain radiographs had improved to Th3.
